JOB SUMMARY

The Family Nurse Practitioner (FNP) provides integrated clinical care across telehealth services and in-person primary care support within the Salus Care Network. This hybrid role combines virtual patient management with on-site participation in preventive care clinics and primary care workflows. All clinical services are delivered under the supervision of a licensed physician, in compliance with Puerto Rico regulations. The FNP does not prescribe medications independently, and all pharmacologic management must be reviewed and authorized by the supervising physician.

ESSENTIAL FUNCTIONS

  • Conduct virtual consultations via video, phone, and asynchronous platforms.
  • Perform clinical assessments, triage, and initial management of acute and chronic conditions.
    Identify patients requiring escalation to physician-level care.
  • Support continuity of care through follow-up telehealth encounters.
  • Support physicians in primary care clinics, including patient intake, clinical evaluation, and care coordination. 
  • Participate in preventive care initiatives (e.g., annual wellness visits, preventive care program, chronic disease screenings, risk assessments).
  • Assist in population health management strategies, including gap closures (HEDIS-style metrics, if applicable).
  • Provide patient education on preventive health, lifestyle modification, and disease management.
  • Collaborate directly with supervising physicians for case discussions, diagnostic and treatment planning, final clinical decision-making. 
  • Escalate cases requiring, prescription medications, diagnostic orders beyond protocol and complex or unstable clinical conditions. 
  •  Document all patient encounters accurately in the EHR in real time.
  • Maintain compliance with Puerto Rico scope-of-practice laws, HIPAA and patient privacy standards and Organizational clinical protocols.
  • Ensure proper documentation to support physician review and co-management when required Deliver culturally competent education tailored to Puerto Rico’s patient population.
  • Reinforce adherence to treatment plans and preventive care guidelines.
  • Support chronic disease management (e.g., diabetes, hypertension, asthma) through both virtual and in-person touchpoints.
  • Meet established productivity benchmarks across both telehealth and in-clinic settings. 
  • Participate in quality improvement and population health initiatives.
  • Contribute to patient satisfaction and access-to-care metrics.

EDUCATION

  • Master's Degree
  • Doctorate Degree

EXPERIENCE

Master’s or Doctoral degree in Nursing (MSN or DNP), Family Nurse Practitioner track. 1 to 3 years of experience in Primary care, urgent care, or ambulatory settings. Experience in telehealth and hybrid care models strongly settings

REQUIRED LICENSE/CERTIFICATIONS
•Active, unrestricted Puerto Rico RN license
•FNP certification (FNP-BC or equivalent)
•Current BLS certification (ACLS preferred)

LICENSES AND CERTIFICATIONS

  • FNP - Family Nurse Practitioner (required)
  • Current / Active Registered Nurse License without restrictions in Puerto Rico (required)
  • BLS - Basic Life Support (required)
